I'm wondering if the CPU isn't seeing all the data bits the ROMs are setting on the data bus. This would make the start address wrong and it would also load a corrupted instruction and maybe try to access something in the I/O area ... OK, so no burst read is being requested but /STERM is being activated. The /BERR happens after a timeout. But /DSACK0 and /DSACK1 never go low! Yes, I'm going to check the solder joints again. It runs great the first 15 minutes, GB6 works and I even could run a demo. But then it starts acting up and it freezing within seconds. I should point out that in general use you'd be hard pressed to spot the difference between a 40 and 50MHz version of the board. During initial development I only had one 50MHz capable CPU but a couple of 40MHz plastic ones so more often than not I was working at 40 and I don't think I'd be able to ...

There's a very nice comment on the video from the snappily named @user-ic6ei6yh9h who reports having five working DSTBs, so the chances are I've just buggered mine up and the original reported problem is indeed just the half-arsed DMA support.

But it'd be nice to be able to prove it. ;)
